Creating a new IEP document

Once all assessments have been completed for a student an Individualized Education Program (IEP) must be developed. The IEP team, which includes the student's parents or guardians, works together to develop and finalize the IEP document.

The IEP document is created in SESIS. A new IEP document must be created for each student who receives special education services every year, or when it is determined that a change must be made to the existing document.

Important: IEPs can also be created in SESIS for Turning 5 students.

To create a new IEP document

  1. Locate the student's profile.
    How?

  2. From the Actions bar, click Documents.

    A list of all documents for the student appears.

  3. From the Create New Document drop-down menu, choose Individualized Education Program (IEP).

  4. Click the Go button.

  5. Click the Create button.

    A new IEP document appears.

  6. Complete all sections of the IEP document. See below for assistance with individual sections.

    Note: For Turning 5 students, the Child Outcomes Summary form must be completed before the IEP can be finalized.
