Completing the IEP: Coordinated Set of Transition Activities section

Each student's Individualized Education Program (IEP) is comprised of many sections. The Coordinated Set of Transition Activities section appears on all IEPs for students who are 14 years or older. This section is automatically included in the student's IEP based on the student's age in the profile.

This section identifies supports that will facilitate the student's transition from school to post-school activities.

Important: Although this is not a required section for students who are younger than 14 years old, the section must not be removed from the IEP document. If the section is unchecked on the Modify Documents set-up page you will not be able to finalize the IEP document. It is not necessary to complete the section for students who are younger than 14.
