Completing the Emotional Disability Justification form
Each student's Individualized Education Program (IEP) is comprised of many sections. The Emotional Disability Justification form is required on IEPs for students whose disability classification, as indicated on the IEP cover page, is Emotional Disability.
This form guides the case manager through a series of questions to determine if Emotional Disability is the appropriate disability classification for the student.
Note: This form only appears on the student's IEP if Emotional Disability is indicated on the Cover page.
To complete the Emotional Disability Justification form
- Locate the student's profile.
How?
- From the Actions bar, click Documents.
A list of all documents for the student appears.
- Click Individualized Education Program (IEP).
The IEP document appears.
Note: If the IEP document does not appear on the list, create a new IEP document.
- From the Actions bar, click More Actions.
- From the pop-up menu, click Modify Document Setup.
The Individualized Education Program (IEP) Setup page appears.
- Check the Emotional Disability Justification Form checkbox.
- Click the Accept button.
- From the Actions bar, click the Select drop-down () icon.
- From the pop-up menu, click Other Sections.
- Click the Emotional Disability Justification Form link.
The form appears in Edit mode.
- Complete the appropriate sections.
- Click the Save, Done Editing button.
