Completing the IEP: Special Transportation/Placement
Recommendation section
Each student's Individualized Education Program (IEP) is comprised of many
sections. The Special Transportation/Placement Recommendation section appears on
all IEPs for all students.
This section identifies whether special transportation or accommodations must
be made for the student. It also identifies the type of placement that is
recommended for the student, such as a community school, a specialized public
school, a state-approved non-public school, a charter school or a day treatment
center.
Note: If a day treatment center is selected, the student must have a
Department of Health and Mental Hygiene (DOHMH) acceptance letter.
To complete the Special Transportation/Placement Recommendation section of the IEP
- Locate the student's documents.
How?
- Click Individualized Education Program (IEP).
The IEP document appears.
Note: If the IEP document does not appear on the list,
create a
new IEP document.
- From the Actions bar, click the Select drop-down () icon.
- From the pop-up menu, click Special Transportation/Placement
Recommendation.
The placement recommendation section of the IEP appears.
- Indicate whether specialized transportation is
needed or not.
Important: If specialized transportation is not needed,
note the following on the Present Levels of Performance Section:
- Do not mark Yes next to Does the child have Limited Mobility?
- Do not mark Yes next to Does the child need
a wheelchair?
- Do not mark Yes next to Does the child use
walking aides?
If any of these fields are marked Yes, SESIS will automatically indicate that
the student needs specialized transportation.
- If specialized transportation is needed, indicate
what accommodations are needed.
Important: Adult Supervision - Paraprofessional can only be
selected if you also choose Paraprofessional on the Recommended Special
Education Programs/Services section.
Note: Certain accommodations (such as air conditioning or
limited travel time) should be approved by the DOHMH physician. If an
approved OSH Physician Review form has not already been faxed into SESIS
using the appropriate fax coversheet, SESIS will provide a workflow prompt
to remind you to have the request approved.
Note: If the student is recommended for Other Accommodations
Carry/Porter Services, a Building Identification Number (BIN) is required.
Look up the BIN online at:
