Completing the Home Instruction Referral form
When the Recommended Special Education Programs/Services section of a student's Individualized Education Program (IEP) indicates that the student needs home instruction, the case manager must request approval from the Office of Home Instruction or the Borough Office of Home Instruction by completing the Home Instruction Referral form in SESIS.
To complete the Home Instruction Referral form
- Locate the student's profile.
How?
- From the Actions bar, click Documents.
A list of all documents for the student appears.
- From the Create New Document drop-down menu, choose Home Instruction Referral Form.
- Click the Go button.
- Click the Create button.
The Home Instruction Referral form appears.
- In the Date field, click the calendar () icon and choose the date of the referral.
- Complete the IEP team sections of the form.
- Click the Save, Done Editing button.
- From the Actions bar, click Change Status of This Document.
- From the Change status from Draft to drop-down menu, choose Review.
- Click the Accept button.
- Click the OK button.
The student appears on the Status of Home Instruction Referrals report as pending review.
