Completing the IEP: Attendance Page section
Each student's Individualized Education Program (IEP) is comprised of many sections. The Attendance Page section appears on all IEPs for all students.
This section identifies all required and optional attendees at the IEP meeting. Individuals that were indicated on the Notice of IEP Meeting will be automatically listed on the Attendance Page.
All attendees must sign the Attendance Page and the signed page must be faxed into SESIS. The IEP cannot be finalized until the Attendance Page has been faxed into the student's SESIS record using the appropriate fax coversheet.
To complete the IEP in SESIS: Attendance Page section
- Locate the student's profile.
How?
- From the Actions bar, click Documents.
A list of all documents for the student appears.
- Click Individualized Education Program (IEP).
The IEP document appears.
Note: If the IEP document does not appear on the list, create a new IEP document.
- From the Actions bar, click the Select drop-down () icon.
- From the pop-up menu, click Attendance Page.
The Attendance Page section of the IEP appears.
- Review the pre-populated names and make revisions if necessary.
- If an individual was not identified on the Notice of IEP Meeting, click the lookup link next to the Name field to add the individual.
- For each participant, indicate if the individual:
- Is bilingual.
- Participated by telephone.
- Attended the meeting.
- Click the Save, Done Editing button.
- From the Actions bar, click the Print link.
- From the pop-up menu, click This Section.
- From the browser's File menu, choose Print.
The Print dialog box appears.
- Choose the appropriate printer and click the Print button.
The Attendance Page prints.
- Have all attendees sign the attendance sheet.
- After the meeting concludes, fax the signed attendance page, using the appropriate fax coversheet, into SESIS.
The IEP cannot be finalized until the Attendance Page has been faxed into the student's SESIS record using the appropriate fax coversheet.
