Completing the 12 Month School Year Consent form
If a student is recommended for a twelve-month special education program, the 12 Month School Year Consent form must be completed by the parent or guardian. The signed form must be faxed into SESIS using the generic fax coversheet.
To complete the 12 Month School Year Consent form
- Locate the student's profile.
How?
- From the Actions bar, click Documents.
A list of all documents for the student appears.
- Click Individualized Education Program (IEP).
The IEP document appears.
Note: If the IEP document does not appear on the list, create a new IEP document.
- From the Actions bar, click More Actions.
- From the pop-up menu, click Modify Document Setup.
The Individualized Education Program (IEP) Setup page appears.
- Check the 12 Months Consent Form checkbox.
- Click the Accept button.
- From the Actions bar, click the Select drop-down () icon.
- From the pop-up menu, click Other Sections.
- Click the 12 Months Consent Form link.
The form appears in Edit mode.
- In the Date filed, click the Calendar () icon and choose today's date.
- Click the Save, Done Editing button.
- Print this section of the IEP.
How?
- Complete the printed copy of the form.
Note: This form cannot be completed electronically.
- Give the form to the parents or guardians and request that they sign the form and return it to you.
- When the parents or guardians return the signed form, fax it into SESIS using the generic fax cover sheet.

Note: When printing the generic fax coversheet, the Category and Form should be Documents Related to IEP.
