Completing the Specific Learning Disability Justification form
Each student's Individualized Education Program (IEP) is comprised of many sections. The Specific Learning Disability form is required on IEPs for students whose disability classification, as indicated on the IEP cover page, is Learning Disability.
This form guides the case manager through a series of questions that help him or her determine if Learning Disability is the appropriate disability classification for the student.
Note: This form only appears on the student's IEP if Learning Disability is indicated on the Cover page.
Tips
- The student's disability must be indicated on the cover page before completing this section.
- The disability must be Learning Disability.
To complete the Specific Learning Disability Justification form
- Locate the student's profile.
How?
- From the Actions bar, click Documents.
A list of all documents for the student appears.
- Click Individualized Education Program (IEP).
The IEP document appears.
Note: If the IEP document does not appear on the list, create a new IEP document.
- Verify that the cover page has been completed and the student's disability has been selected.
Note: This will automatically add the section to the Select drop-down menu.
- From the Actions bar, click the Select drop-down () icon.
- From the pop-up menu, click Specific Learning Disability Justification Form.
The form appears in Edit mode.
- Complete the appropriate sections.
- Click the Save, Done Editing button.
- Upon completion of this section, follow the guided action to return to the cover page or choose the next section you wish to complete.
