Completing an Additional Request for Reevaluation

One reevaluation of an Individualized Education Program (IEP) per school year may occur without approval. A request for a subsequent reevaluation within the same school year must be approved by the principal or the Committee on Special Education (CSE) chairperson.

To complete an Additional Request for Reevaluation

  1. Verify that a reevaluation has already been conducted during the current school year.

    Note: If a reevaluation has not been conducted this school year, approval is not required. You may create a Request for Reevaluation.

  2. Locate the student's profile.
    How?

  3. From the Actions bar, click Documents.

    A list of all documents for the student appears.

  4. From the Create New Document drop-down menu, choose Additional Request for Reevaluation.

  5. Click the Go button.

  6. Click the Create button.

    The Request for Reevaluation page appears.

  7. Click the calendar () icon next to the Date field and choose today's date.

  8. Click the Save, Done Editing button.

  9. Determine who made the request for the additional reevaluation.

  10. Select from below.

    If the request was made by... Do this...
    Principal or CSE chairperson
    1. Send the Notice of Request for Reevaluation to the parent or guardian.
      How?

    2. If additional testing is required, send the Consent for Additional Assessments letter.

    Parent or guardian
    1. Print the fax coversheet.
      How?

    2. Fax the signed consent form into SESIS.
      How?

    3. Submit the Additional Request for Reevaluation to the principal for approval.
      How?
    Anyone else
    1. Submit the Additional Request for Reevaluation to the principal or CSE chairperson for approval.
      How?
