Creating a Request for Reevaluation

A student receiving services mandated by an Individualized Education Program (IEP) must be reevaluated at least once every three years. A reevaluation may occur sooner than three years if necessary.

Referrals for reevaluation may be made under the following circumstances:

Important: A reevaluation may occur once per school year without approval. Any subsequent reevaluations per school year must be approved by the principal or Committee on Special Education (CSE) chairperson. In that case, complete an Additional Request for Reevaluation instead.

To create a Request for Reevaluation

  1. Determine if a reevaluation has been conducted within the school year.

  2. Select from below.

    If... Do this...
    A reevaluation has already been conducted this school year All subsequent requests for reevaluation must be submitted for approval.
    1. Complete an Additional Request for Reevaluation.
      How?
    A reevaluation has not been conducted this school year Approval is not required for the first reevaluation in a school year.
    1. From the Actions bar, click Documents.

      A list of all documents for the student appears.

    2. From the Create New Document drop-down menu, choose Request for Reevaluation.

    3. Click the Go button.

    4. Click the Create button.

      The Request for Reevaluation page appears.

    5. Click the calendar () icon next to the Date field and choose today's date.

    6. In the Academics field, type the appropriate information.

    7. In the Social/Behavior Development field, type the appropriate information.

    8. In the next section, click the Insert Statements link.

    9. Review the list and locate the reason for requesting a reevaluation.

    10. Click the appropriate statement.

      The strikethrough on the statement indicates that it has been selected and inserted in the field.

      Note: You may edit the statement once it is inserted into the field.

    11. Click the Close window link.

    12. Indicate whether the student is being considered for a change in program recommendation.

    13. Click the Save, Done Editing button.
