Sending the Consent for Additional Assessment (New Testing Needed) letter
When a reevaluation of a student's Individualized Education Program (IEP) occurs, additional assessments may be needed. If this is the case, the case manager must request consent from the parents or guardians for the additional assessments.
If the parents or guardians do not respond to the request for consent, the case manager may proceed with the assessments after two outreach attempts have been logged in SESIS.
To send the Consent for Additional Assessment (New Testing Needed) letter
- Locate the student's profile.
How?
- From the Actions bar, click Documents.
A list of all documents for the student appears.
- Click Assessment Planning.
- Verify that the New testing needed check box is checked.
Note: If the check box is not checked, from the Actions bar, click Edit this section and check the box.
- From the Actions bar, click the Select drop-down () icon.
- From the pop-up menu, click Consent for Additional Assessments.
Note: If Consent for Additional Assessments is not on the pop-up menu, do the following:
- From the Actions bar, click More Actions.
- From the pop-up menu, choose Modify Document Setup.
The Document Setup window appears.
- Check the Consent for Additional Assessments checkbox.
- Click the Accept button.
- From the Actions bar, click the Select drop-down () icon.
- From the pop-up menu, click Consent for Additional Assessments.
- From the Actions bar, click Edit this section.
- Verify that the date in the Date field matches the date the letter will be mailed. The default is today's date. If the notice will not be mailed today, click the calendar () icon next to the Date field and choose the appropriate date.
Note: The date you type in this field will be recorded as the date the letter is sent.
- At the bottom of the letter, click the calendar () icon and choose a date by which the parent or guardian should respond to the letter.
Note: This date must be 10 or more days from the date of the letter.
- Click the Save, Done Editing button.
- From the Actions bar, click Send/Print Letter.
- Click OK to record sending the letter as an event.
The event log appears.
Note: The send/print date is automatically recorded and the Description field is auto-populated. No other fields are required.
- Click the Accept/Print button.
The Print dialog box appears.
- Select the appropriate printer and click the Print button.
The document prints.
- Click the Back to previous screen link.
- From the Actions bar, click More Actions.
- Finalize the document.
