Submitting an Additional Request for Reevaluation for approval

One reevaluation of an Individualized Education Program (IEP) per school year may occur without approval. A request for a subsequent reevaluation within the same school year must be approved by the principal or the Committee on Special Education (CSE) chairperson.

To submit an Additional Request for Reevaluation for approval

  1. Determine if there has been a reevaluation within the past school year.

  2. Select from below.

    If... Do this...
    There has been one or more reevaluation within the past school year All subsequent requests for reevaluation must be submitted for approval.
    1. Create the Additional Request for Reevaluation.
      How?

    2. View the Additional Request for Reevaluation document.
      How?

    3. From the Actions bar, click More Actions.

    4. From the pop-up menu, click Change the Status of this Document.

    5. From the Change status from Draft to drop-down menu, choose Review.

    6. Click the Accept button.

    7. Click the OK button to change the status of the document to review.

      The request is submitted to the principal or CSE chairperson for approval.
    There has not been a reevaluation within the past school year Approval is not required for the first reevaluation for the year.
