Completing evaluations

When a student's parent or guardian provides consent for evaluation for special education services, the Assessment Planning document is created to outline the specific evaluations that are needed. All evaluations identified in the Assessment Planning document must be completed and finalized in SESIS.

Most evaluations have an evaluation document template in SESIS. The following evaluation templates are available in SESIS:

For all other evaluation types, the generic assessment is used.

Important: Do not use the generic assessment template for evaluations that have a custom template in SESIS. The Assessment Planning document will not recognize the evaluation if it is not completed using the correct template.

To complete evaluations

  1. Locate the student's profile.
    How?

  2. From the Actions bar, click Documents.

  3. From the Create New Document drop-down menu, choose the appropriate evaluation template from the Assessment section.

    Note: If a custom evaluation template does not exist in SESIS for the evaluation type, choose Generic (for all other assessments).

  4. Click the Go button, then click the Create button.

    The evaluation template appears.

  5. Complete the document, ensuring that all required information is completed.

  6. If applicable, print the fax coversheet and fax in any supporting documentation.

  7. Note: If the evaluation was conducted outside of SESIS, print the fax coversheet and fax the paper evaluation into SESIS.

  8. Finalize the document.
