Creating the Assessment Planning document in SESIS

When the student's parent or guardian has provided consent for evaluation for special education services, the Assessment Planning document must be created. This document outlines the specific assessments that are needed for the student. The individual assessments required depend on the student's needs, however, all initial cases include the following assessments:

When a Request for Reevaluation document has been finalized in SESIS the student's process status is updated to Reevaluation. The Assessment Planning document differs slightly for a reevaluation than for an initial referral.

To create the Assessment Planning document in SESIS

  1. Select from below.
    If... Do this...
    Initial referral with consent received.
    1. Locate the student's Documents page.
      How?

      Important: Do not create the Assessment Planning document unless the Consent for Initial Evaluations appears as an attachment to the most recent Social History Package.
    2. From the Create New Document drop-down menu, choose Assessment Planning.

    3. For each required assessment, do the following:

      1. From the Assessments Needed drop-down menu, choose the appropriate assessment.

        Note: The Social History assessment is pre-populated because it was created as part of the Social History Package.

        Tip: For help determining what assessments are needed, click the Access ARIS link to access data from ARIS.

      2. From the Evaluator drop-down menu, choose the evaluator type.
      3. Click the Save, Done Editing button.

      4. If applicable, notify the appropriate school staff of any pending evaluations to be conducted.
    Initial referral: consent not received.
    1. Close the case

      How?

    Initial referral: parent refused to consent.
    1. Make another outreach attempt. Do not create the Assessment Planning document until consent has been obtained.
    Re-evaluation with no new testing needed.
    1. In SESIS, locate the student's Documents page.

    2. From the Create New Document drop-down menu, choose Assessment Planning.

    3. Click the Go button.

    4. Click the Create button.

    5. On the Assessment Planning document, check the No checkbox next to New testing needed.

    6. Click the Save, Done Editing button.
    7. Send the Notice of No New Testing Needed letter.
      How?

    8. Finalize the Assessment Planning document.
      How?

    9. Send the Notice of IEP Meeting.
      How?

    Re-evaluation with new testing needed.
    1. On the Assessment Planning document, check the Yes checkbox next to New testing needed.

    2. Type a reason for the new testing.

    3. Click the Save, Done Editing button.

    4. Send the Consent for Additional Assessment (New Testing Needed) letter.
      How?

Note: The process is the same for reevaluations and mandated three-year reevaluations.
