Finalizing the Assessment Planning document
When a student's parent or guardian provides consent for evaluation for special education services, the Assessment Planning document is created to outline which assessments are needed. All assessments identified in the document must be completed and finalized in SESIS before the Assessment Planning document can be finalized.
Important: The Assessment Planning document must be finalized before the Individualized Education Program (IEP) is finalized. It cannot be held open for a late assessment and finalized later.
To finalize the Assessment Planning document
- Locate the student's profile.
How?
- From the Actions bar, click Documents.
A list of all documents for the student appears.
- Click Assessment Planning.
Note: If the Assessment Planning document does not appear on the list, create the Assessment Planning document.
- The Assessment Planning document appears.
- In the Assessments Needed table, verify that there is a date in each Date Completed field.
Note: If all evaluations do not have a date in the Date Completed field, complete and finalize all remaining evaluations.
Important: The Assessment Planning document cannot be finalized until all identified assessments have been completed and finalized in SESIS.
- Confirm the following:
- All evaluations have been completed in SESIS using the appropriate document template.
- If appropriate, any supporting documentation has been faxed into SESIS using the appropriate fax coversheet.
- From the Actions bar, click More Actions.
- From the pop-up menu, click Change Status of This Document.
- From the Change Status from Draft to drop-down menu, choose Final.
- Complete the remaining fields if appropriate.
- Click the Accept button.
The Assessment Planning document is finalized.
