Completing the Monitoring Blood Sugar, Parent Consent form

Students who need to monitor their blood sugar while at school must have a completed Monitoring Blood Sugar, Parent Consent form in their SESIS record. The consent form is required whether services are provided by school personnel or by the student.

Important: Medical forms are not completed in SESIS. The form must be printed and given to the parent or guardian to complete. The completed form must be faxed into SESIS using the appropriate fax coversheet.

To complete the Monitoring Blood Sugar, Parent Consent form

1. Verify that the parent or guardian has returned the completed form.
   
   Note: If the parent or guardian has NOT returned the completed form yet, print and send the form to the parent or guardian. When you receive the completed form, return to this procedure.
   
   
2. Locate the student's profile.
   How?
   
   
3. Click the Documents () icon.
   
   
4. From the Create New Document drop-down menu, choose Medical Forms under the IEP section.
   
   
5. Click the Go button.
   
   
6. Click the Create button.
   
   The Diabetes Medication Administration form appears.
   
   
7. If school personnel must administer diabetes medication, complete the Diabetes Medication Administration form.
   How?
   
   
8. From the Actions bar, click the Select drop-down () icon.
   
   
9. From the pop-up menu, click Monitoring Blood Sugar, Parent Consent.
   
   The Monitoring Blood Sugar, Parent Consent form appears.
   
   
10. If the document is in Edit mode, click the Cancel button.
    
    
11. From the Actions bar, click Print.
    
    
12. From the pop-up menu, click FAX Coversheet.
    
    
13. From the Category drop-down menu, choose IEP - Medical.
    
    
14. From the Form drop-down menu, choose Monitoring Blood Sugar Medication and Dietary Needs: Parent/Guardian Consent and Authorization.
    
    
15. Click the calendar () icon next to the Date Received field and choose the date the completed form was received.
    
    
16. Click the calendar () icon next to the Date of Signature field and choose the date the form was signed by the parent or guardian.
    
    
17. Click the Print button.
    
    The fax coversheet appears in Adobe Acrobat.
    
    
18. In Acrobat, click the print () icon.
    
    The Print dialog box appears.
    
    
19. Choose the appropriate printer and click the OK button.
    
    The fax coversheet prints.
    
    
20. Fax the coversheet and the signed physical examination form to the number provided on the coversheet.
    
    
21. Finalize the document.
