Printing blank medical forms from SESIS for a parent or guardian

Some students require the administration of medication during school hours. All students who receive special education services and who require the administration of medication during school hours must have a completed Medication Administration form in their SESIS record. The completed form must be faxed into SESIS using the appropriate fax coversheet.

Important: Medical forms are not completed in SESIS. The form must be printed and given to the parent or guardian to complete. The completed form must be faxed into SESIS using the appropriate fax coversheet.

To print blank medical forms from SESIS for a parent or guardian

1. Locate the student's profile.

2. Click the Documents () icon.
   
   
3. From the Actions bar, click Print a Blank Document.
   
   
4. From the Select a Template drop-down menu, choose Medical Forms under IEP.
   
   
5. Check the box next to each of the forms you want to print.
   
   
6. Click the OK button.
   
   The selected forms appear.
   
   
7. Click the Print this document link.
   
   The Print dialog box appears.
   
   
8. Choose the appropriate printer and click the Print button.
   
   The blank forms print.
   
   
9. Click the Back to previous screen link.
   
   
10. Create a manual event to record having printed and sent the blank forms to the parent or guardian.
