Completing the Medication Administration form

Some students require the administration of medication during school hours. All students who receive special education services and who require the administration of medication during school hours must have a completed Medication Administration form in their SESIS record. The completed form must be faxed into SESIS using the appropriate fax coversheet.

Note: If the medication is for diabetes, the Diabetes Medication Administration form must be completed instead.

Important: Medical forms are not completed in SESIS. The form must be printed and given to the parent or guardian to complete. The completed form must be faxed into SESIS using the appropriate fax coversheet.

To complete the Medication Administration form

  1. Verify that the parent or guardian has returned the completed form.

    Note: If the parent or guardian has NOT returned the completed form yet, print and send the form to the parent or guardian. When you receive the completed form, return to this procedure.

  2. Locate the student's profile.
    How?

  3. Click the Documents () icon.

  4. From the Create New Document drop-down menu, choose Medical Forms under the IEP section.

  5. Click the Go button.

  6. Click the Create button.

    The Diabetes Medication Administration form appears.

  7. Click the Cancel button.

  8. From the Actions bar, click the Select drop-down () icon.

  9. From the pop-up menu, click Medication Administration Form.

  10. From the Actions bar, click Print.

  11. From the pop-up menu, click FAX Coversheet.

    The fax coversheet page appears.

  12. From the Category drop-down menu, choose IEP - Medical.

  13. From the Form drop-down menu, choose Medication Administration Form.

  14. Click the calendar () icon next to the Date Received field and choose the date the completed form was received.

  15. Click the calendar () icon next to the Date of Signature field and choose the date the completed form was signed by the parent or guardian.

  16. Click the Print button.

    The fax coversheet appears in Adobe Acrobat.

  17. In Acrobat, click the print () icon.

    The Print dialog box appears.

  18. Choose the appropriate printer and click the OK button.

    The fax coversheet prints.

  19. Fax the coversheet and the signed Medication Administration form to the number provided on the coversheet.

  20. Finalize the document.
