Completing the Diabetes Medication Administration form
If a student with diabetes requires assistance administering insulin or other diabetes medications while at school, the Diabetes Medication Administration form must be completed. The form must be completed and signed on paper and faxed into SESIS using the appropriate fax coversheet.
Note: If the Diabetes Medication Administration form is completed, the Medication Administration form is not required unless the student requires additional medication not related to diabetes.
Important: Medical forms are not completed in SESIS. The form must be printed and given to the parent or guardian to complete. The completed form must be faxed into SESIS using the appropriate fax coversheet.
To complete the Diabetes Medication Administration form
- Verify that the parent or guardian has returned the completed form.
Note: If the parent or guardian has NOT returned the completed form yet, print and send the form to the parent or guardian. When you receive the completed form, return to this procedure.
- Locate the student's profile.
How?
- Click the Documents (
) icon.
- From the Create New Document drop-down menu, choose Medical Forms under the IEP section.
- Click the Go button.
- Click the Create button.
The Diabetes Medication Administration form appears.
- Click the Save, Done Editing button.
- From the Actions bar, click Print.
- From the pop-up menu, click FAX Coversheet.
The fax coversheet page appears.
- From the Category drop-down menu, choose IEP - Medical.
- From the Form drop-down menu, choose NYC Office of School Health - Diabetes Medication Administration Form.
- Click the calendar (
) icon next to the Date Received field and choose the date the completed form was received.
- Type the name of the practitioner
- Click the Print button.
The fax coversheet appears in Adobe Acrobat.
- In Acrobat, click the print (
) icon.
The Print dialog box appears.
- Choose the appropriate printer and click the OK button.
The fax coversheet prints.
- Fax the coversheet and the signed physical examination form to the number provided on the coversheet.
- Complete the Monitoring Blood Sugar, Parent Consent form.
How?
- If the student requires the administration of other medication not related to diabetes, complete the Medication Administration form.
