Completing the Prior Notice Package (PNP) for Placement
When an Individualized Education Program (IEP) is finalized,
you must create and send the Prior Notice Package (PNP) for Placement
to the parent or guardian. The package will contain one or more sections,
depending on factors such as the student’s process stage, the type of placement being recommended, etc.
The possible sections for any Prior Notice Package for Placement are:
- Prior Written Notice (PWN): a summary of the student’s IEP recommendations.
- School Location Letter: identifies the school location that
the student is assigned to attend, if the student is changing schools. The
School Location Letter is automatically included when the student has been
placed in a new school.
Placement staff associated with the Office of Student Enrollment and District 75
will add this section manually, if needed.
- Consent for Initial Provision of Services: For initial
cases, the parent or guardian must consent to the placement indicated in the
Prior Written Notice by signing and returning this form. The student cannot receive special
education services until signed consent is received.
This form is not required for annual reviews or reevaluations.
- Consent for Services/Deferred Placement: When the student
is recommended for deferred services, the parent or guardian must consent to the
deferment. This is true for all process stages.
To create the Prior Notice Package (PNP) for Placement
- Locate the student’s documents.
How?
- Verify that the most recent IEP has been finalized.
Important: Do not create the PNP or place
the student until the IEP is finalized.
- From the Create New Document drop-down menu, choose
Prior Notice Package for Placement.
- Click the Go button, then click the Create button.
The Prior Written Notice (PWN) section of the PNP appears in
edit mode.
- Review each section of the document for accuracy.
- Most sections of the PWN are
automatically completed based on the
information in the student’s finalized IEP
and Assessment Planning document.
- At least one document must be indicated in the Evaluation Procedure/Assessment/Record/Report section for the PNP to be finalized.
Note: Rarely, you may
need to add assessments, reports, or other
materials that were provided after the
Assessment Planning document was finalized.
These should be added using the Evaluation
Procedure/Assessment/Record/Report drop-down
menu.
- Complete any remaining required sections using the
lookup links, text fields, checkboxes, calendar
icons and prompts.
- To indicate on the printed letter what is being included
in the package, check the checkboxes next to any form(s) to
be sent to the parent or guardian:
- Parent Consent Form: if it is an initial
case or deferred placement.
- School Location Form: if it is an
initial case or the services will
be provided at a new location.
Important: Checking these boxes does
not cause these sections to automatically
print. To
print these sections, click the Select drop-down (
)
icon and select the desired form. If the Select drop-down
icon is not visible, no additional sections are available.
- Click the Save, Done Editing button.
- If necessary, review the following additional
sections of the PNP.
- If this is an initial case, review the
Consent for Initial Provision of Services.
How?
- If the student is recommended for
deferred placement, review the Consent for
Services/Deferred Placement.
How?
- If this is an initial case or the student will be moving to a new
location, review the School Location
Letter.
How?
- Finalize the document.
How?
Important: The PNP must be finalized before it is printed
and given to the parent or guardian.
- Print each applicable section of the PNP.
- From the Actions bar, click the Select drop-down (
) icon.
Note: If the Select drop-down icon is not visible, no additional sections are available. Proceed to
step 3.
- Select Prior Notice Package for Placement.
The finalized Prior Notice Package for
Placement appears.
- From the Actions bar, click Send/Print
Letter.
- Complete the appropriate fields to record
sending the letter as an event in the
student's event log.
Important: If the PNP will
not be mailed to the parent/guardian, edit
the Event Description text box to indicate
the actual form of delivery used, for
example "Gave to parent at the end of the
IEP meeting."
- Click Accept/Print.
- The print dialog box appears.
- Choose the appropriate printer and click the
Print button.
- Click the Back to previous screen link at
the top of the page.
- If other sections, such as the School
Location Letter, Consent for Initial
Services, or Consent for Services/Deferred
Placement, are included in the Prior Notice
Package, select each section and repeat the
steps to print the individual sections.
Important: The additional
sections will not print automatically.
- Provide a copy of the PNP to the parent or guardian.
