Handling a Turning 5 case

All Turning 5 initial referrals and reevaluations will be opened in CAP. Once the referral is opened in CAP, Turning 5 students will appear in SESIS with an open process stage of Initial or Reevaluation.

Important: School-based and Committee on Special Education (CSE) Individualized Education Program (IEP) teams should not create the Notice of Initial Referral, Notice of Referral, or the Request for Reevaluation documents in SESIS. All Turning 5 referrals must be opened in CAP.

To handle a Turning 5 case

  1. Determine if the case is an initial referral or a reevaluation.

  2. Select from below.

    If... Do this...
    Initial referral
    1. Proceed to step 3.
    Reevaluation All reevaluations for existing turning 5 students have already been opened and are ready for processing in SESIS. The student appears in SESIS with a process stage as follows:

    • Process Stage: Reevaluation

    • Process Type: Referral

    • Is this process open? Yes

    1. Create the Assessment Planning document.
      How?

      Important: No further work is to be completed in CAP.

    2. Complete all assessments and finalize the Assessment Planning document.

    3. Create the Notice of IEP Meeting.
      How?

    4. Complete the IEP.
      How?

    5. Proceed to step 5.

  3. Determine if the referral has been opened in CAP.

  4. Select from below.

    If... Do this...
    Yes, the referral has been opened in CAP but it has been less than one week
    1. Wait at least one week for the case to appear in SESIS.
    Yes, the referral has been opened in CAP and at least one week has passed
    1. Send the Notice of Social History.
      How?

      Important: No further work is to be completed in CAP.

    2. Create the Assessment Planning document.
      How?

    3. Complete all assessments and finalize the Assessment Planning document.

    4. Create the Notice of IEP Meeting.
      How?

    5. Complete the IEP.
      How?

    6. Proceed to step 5.
    No, the referral has not been opened in CAP
    1. In CAP, enter the initial referral.

      Note: Turning 5 initial referral cases will migrate to SESIS weekly.

    2. Wait one week. After one week, the student will appear in SESIS with a process stage as follows:

      • Process Stage: Initial

      • Process Type: Referral

      • Is this process open? Yes

  5. Complete the Cover Page and be sure to choose an appropriate school-aged disability classification in the Disability Classification section.

    Important: If the IEP is finalized with the default classification of "Pre-school student with a Disability," the IEP will not be valid and the data will not flow back to CAP.

  6. In the Recommended Special Education Programs and Services section, complete all required fields for the programs, related services, and/or supplemental services recommended for the student.

  7. Choose each appropriate testing accommodation one time only.

  8. Verify that the Date of IEP Meeting is not future dated but that it is dated after the date of referral.

  9. If the case is a Reevaluation, create and finalize the Child Outcome Summary form.
    How?

    Important: The Child Outcome Summary form is not required for Initial cases.

  10. Finalize the IEP.
