Updating a students Social History package in SESIS

An initial social history is conducted as part of the initial referral process. Information is gathered about the student including languages spoken at home, any concerns the parents or guardians may have about the student, the student's performance at home and what types of strategies have been successful in the past.

In SESIS, the social history can only be updated in the course of a re-evaluation by following the steps below.

To update a students Social History package in SESIS

  1. In SESIS, locate the student's profile.
    How?

  2. From the Actions bar, click Documents.

  3. Click Assessment Planning.

    The Assessment Planning document appears.

    Note: If the Assessment Planning document is not on the list of documents for the student, create the document.

  4. From the Actions bar, click Edit this section.

  5. From the Assessments Needed drop-down menu, choose Other.

  6. In the Please specify field, type Social History Update.

  7. From the Evaluator drop-down menu, choose the appropriate evaluator.

  8. Click the Save, Done Editing button.

  9. From the Actions bar, click the student's name.

    A list of all documents for the student appears.

    Note: If the Documents page does not appear, click Documents from the Actions bar.

  10. From the Create New Document drop-down menu, choose Generic (for all other assessments).

  11. Click the Go button.

  12. In the Comment field, type Social History update.

  13. Click the Create button.

    The generic assessment page appears.

  14. In the Please Specify field, type Social History update if it is not already there.

  15. Complete all relevant fields.

  16. Click the Save, Done Editing button.

  17. If no further changes are needed, finalize the document.
