Social History package

The Social History package consists of all of the documents needed for the social worker to conduct the social history meeting with the parent or guardian.

The package includes the following documents:

Some of the documents in the package are completed by the parent or guardian and others are completed by the student's physician, guidance counselor, or other school official.
