Process overview: Mandated three-year reevaluation

All Individualized Education Programs (IEPs) must be reevaluated at least once every three years.

Note: In some cases, the parent or guardian and the principal may choose to waive the mandated three-year reevaluation.

Process overview: Mandated three-year reevaluation

  1. A DOE staff member creates the Mandated Three-Year Reevaluation document in SESIS.

  2. A DOE staff member finalizes and sends the Notice of Mandated Three-Year Reevaluation to the parent or guardian.

    If... Do this...
    The principal and parent or guardian agree to waive the mandated three-year reevaluation
    1. A DOE staff member sends the Request for Waiver of Mandated Three-Year Reevaluation to the parent or guardian.

    2. The parent or guardian signs the form and returns it to the school.

    3. A DOE staff member prints the Request for Waiver of Mandated Three-Year Reevaluation fax coversheet and faxes the signed waiver along with the coversheet into SESIS.

    4. The mandated three-year reevaluation process stops. The existing IEP remains in effect.
    The principal and parent or guardian do not agree to waive the reevaluation
    1. A DOE staff member creates the Assessment Planning document.

    2. A DOE staff member indicates if new testing is needed.

    3. Proceed to step 4.

    If... Do this...
    New testing is needed
    1. A DOE staff member prints and sends the Consent for Additional Assessments form to the parent or guardian.

    2. The parent or guardian returns the signed the Consent for Additional Assessments form.

      Important: For reevaluations, passive consent is acceptable as long as the DOE staff member documents three outreach attempts in SESIS.

    3. A DOE staff member prints the Consent for Additional Assessments (New Testing Needed) fax coversheet and faxes the signed consent along with the coversheet into SESIS.

    4. The case manager adds the appropriate assessments to the Assessment Planning document and does one of the following:

      • Indicates that the evaluation will be completed by school staff.

      • Requests assistance from the Children First Network (CFN).

      • Indicates that prior assessments will be used.

    5. The evaluators complete all evaluations with the student.

    6. Each evaluator creates a document in SESIS to record his or her evaluation.

    7. Once all evaluations are completed and finalized in SESIS, the case manager finalizes the Assessment Planning document.

      Important: The case manager may not finalize the Assessment Planning document until all evaluations are entered in SESIS and finalized.

    8. Proceed to step 5.
    New testing is not needed
    1. A DOE staff member creates and finalizes the Notice of No New Testing letter and sends it to the parent or guardian.

    2. A DOE staff member finalizes the Assessment Planning document.

    3. Proceed to step 5.

  5. The case manager creates, finalizes, and sends the Notice of IEP meeting to the parent or guardian.

  6. The case manager creates the IEP document in SESIS.

  7. The case manager prints the IEP meeting Attendance Page.

  8. The IEP team conducts the IEP meeting and each participant signs the Attendance Page.

  9. The case manager prints the Attendance Page fax coversheet and faxes the coversheet along with the signed Attendance Page.

  10. The case manager makes any necessary adjustments to the IEP based on decisions made during the IEP meeting.

  11. The case manager finalizes the IEP document.

  12. The case manager sends a copy of the finalized IEP to the parent or guardian.

  13. The case manager places the student using CAP/SEC.
