Completing the Home Language Identification Survey (HLIS) in SESIS

The Home Language Identification Survey (HLIS) verifies the language that the student speaks at home. If the student's primary language is not English, the multidisciplinary assessment must include the student's primary language.

For students who attend a New York City public school, this information is pulled into SESIS from ATS. If it has migrated correctly, the Home Language Identification Survey checkbox on the student's Profile page will be checked. For non-attending students and students whose SESIS profile is missing this information, the survey must be completed in SESIS as part of the Social History package.

Important: For public school students, if updates to the student's or parent's home language are necessary, then those updates must be made in ATS.

To complete the HLIS

    1. Locate the student's Profile.
      How?

    2. Determine that the checkbox next to Home Language Identification Survey is unchecked to verify that the HLIS has not already been completed.

      Tip: If the box is checked, but you are unable to complete the Social History package and you see an error message saying the HLIS is missing, re-fax the HLIS using the Fax Coversheet from the Social History package.

    3. From the Actions bar, click Documents.

      A list of all documents for the student appears.

    4. Click Social History Package.

    5. From the Actions bar, click the Select drop-down () icon.

    6. From the pop-up menu, click Home Language Identification Survey.

      The Home Language Identification Survey appears.

    7. Click the calendar () icon next to the Date field and select the date the survey was completed.

    8. Determine if the parent or guardian completed and signed the paper form.

    9. If... Do this...
      Yes, the parent or guardian completed and signed the paper form
      1. Click the Save, Done Editing button.

      2. From the Actions bar, click Print.

      3. From the pop-up menu, choose Fax Coversheet.

      4. From the Category drop-down menu, choose Social History Package.

      5. From the Form drop-down menu, choose Home Language Identification Survey.

      6. Click the calendar () icon and select the date that the signed form was received.

      7. From the Parents Home Language drop-down menu, choose the language that the parents or guardians speak at home.

      8. Click the Print button.

        The fax coversheet appears in Adobe Acrobat.

      9. In Acrobat, click the print () icon.

        The Print dialog box appears.

      10. Choose the appropriate printer, then click the OK button.

      11. In SESIS, click the browser back button.

        Note: This is the only situation where it is OK to click the back button in SESIS.

      12. From the Actions bar, click Social History Package.

        The Home Language Identification Survey appears.

      13. Complete the remaining sections of the Social History Package.
      No, the parent or guardian did not complete the paper form
      1. From the Actions bar, click Edit This Section.

      2. Complete all fields.

      3. Click the Save, Done Editing button.
