Completing the Student Ethnicity Identification form
The New York City Department of Education (DOE) has adopted a policy that requires the collection and recording of the ethnic identity of students in the New York City public school system. The information is used to do the following:
- Plan educational programs and make sure that they are readily available to all students.
- Study the movement of students in different ethnic groups as they move from school to school.
- Analyze differences in academic performance, attendance, and completion of school.
All information is kept confidential.
For students who attend a New York City public school, this information is pulled into SESIS from ATS. For non-attending students and students whose SESIS profile is missing this information, the form must be completed in SESIS as part of the Social History package.
To complete the Student Ethnicity Identification form
- Locate the student's profile.
How?
- From the Actions bar, click Documents.
A list of all documents for the student appears.
- Click Social History Package.
- From the Actions bar, click the Select drop-down () icon.
- From the pop-up menu, choose Student Ethnicity Identification.
The Student Ethnicity Identification form appears.
- Determine if the parent or guardian completed and signed the paper form.
- Select from below.

- From the Actions bar, click Print.
- From the pop-up menu, choose Fax Coversheet.
- From the Category drop-down menu, choose Social History Package.
- From the Form drop-down menu, choose Parent Guardian Student Ethnic Identification.
- Click the calendar () icon and select the date that the signed form was received.
- Check the appropriate ethnicity checkbox.
- Click the Print button.
The fax coversheet appears in Adobe Acrobat.
- In Acrobat, click the print () icon.
The Print dialog box appears.
- Choose the appropriate printer, then click the OK button.
- In SESIS, click the browser back button.
Note: This is the only situation where it is OK to click the back button in SESIS.
- From the Actions bar, click Social History Package.
The Student Ethnicity Identification form appears.
- Complete the remaining sections of the Social History package.

- From the Actions bar, click Edit This Section.
- Complete all fields.
- Click the Save, Done Editing button.
- Complete the remaining sections of the Social History package.
