Completing the School Placement Request to OSE/D75
When a student is recommended for special educations services, you must find
an appropriate class within the student's current school. If his or her current
school cannot offer an appropriate class, you may request placement in a new school. To
do this, you must complete a School Placement Request to OSE/D75 in SESIS. The
Office of Student Enrollment (OSE) or District 75 (D75) Central must review the
request and, if approved, place the student.
To complete the School Placement Request to OSE/D75
- Locate the student’s documents.
How?
- From the Create New Document drop-down menu, choose
School Placement Request to OSE/D75.
- Click the Go Button, then click the Create button.
The School Placement Request to OSE/D75 appears.
- From the School Placement Request to OSE/D75 drop-down,
choose either OSE or D75.
- Click the lookup link next to the School field.
- On the search form, search for the requested school.
- From the list of search results, click the Select button
next to the appropriate school.
The school name appears in the School field.
- Complete all other relevant fields.
- Click the Save, Done Editing button.
- From the Actions bar, click More Actions.
- From the pop-up menu, click Change Status of This
Document.
- From the “Change status from Draft to” drop-down menu,
choose Review.
- Click the Accept button, and then click the OK button.
- OSE or D75 will review the form and accept or reject the
request, or request more information from the school.
- Monitor the Placement Status
report. The Request Status column will alert you to
updates from OSE or D75, including requests for more
information.
