Completing the Travel Training Evaluation Referral form

If the Individualized Education Program (IEP) team believes that a student attending a community school meets the criteria for travel training, then a travel training referral form must be completed in SESIS and submitted for review and approval to the Office of Travel Training.

To complete the Travel Training Evaluation Referral form

  1. Locate the student's profile.
    How?

  2. From the Actions bar, click Documents.

    The Documents page appears.

  3. From the Create New Document drop-down menu, choose Travel Training Evaluation Referral.

  4. Click the Go button, then click the Create button.

    The Travel Training Evaluation Referral form appears.

  5. Type all required information.

    Tip: Required fields are highlighted in pink. They turn to yellow when they have been completed.

  6. Click the Save, Done Editing button.

  7. From the Actions bar, click More Actions.

  8. From the pop-up menu, choose Change Status of This Document.

  9. From the Change Status from Draft to drop-down menu, choose Review.

  10. Complete the additional fields, if appropriate.

  11. Click the Accept button.

    The Office of Travel Training will review the request.

  12. Review the Status of Travel Training Evaluation Referrals report for the status of this referral.
