Submitting a referral for an Assistive Technology evaluation in SESIS
When the Individualized Education Program (IEP) team determines that a student requires an assistive technology evaluation, the IEP team must add the evaluation to the student's Assessment Planning document. If there is not a New York City Department of Education (DOE) staff member at the school who is qualified to conduct the evaluation, the IEP team must submit a referral for an evaluation as follows:
- Students attending a District 75 program must be referred to the Office of Technology Solutions (OTS).
- All other students must be referred to the Center of Assistive Technology (CAT).
To submit a referral for an Assistive Technology evaluation in SESIS
- In SESIS, locate the student's profile.
How?
- From the Actions bar, click Documents.
A list of all documents for the student appears.
- From the Create New Document drop-down menu, choose the appropriate document:
- For students attending a District 75 program: Choose Assistive Technology Evaluation
Referral (D75).
- For all other students: Choose Assistive Technology Evaluation
Referral (non-D75)
- Click the Go button, then click the Create button.
The Request for Assistive Technology Evaluation form appears.
- Complete all relevant details.
- Click the Save, Done Editing button.
- From the Actions bar, click Change Status of This Document.
- From the Change Status from Draft to drop-down menu, choose Review.
- Click the Accept button.
- Note the following:
- The referral will be added to the appropriate status report.
- OTS or CAT will review the request and will indicate their decision in SESIS.
- If the request for an Assistive Technology referral is approved, the IEP team must
complete an Assistive Technology evaluation in SESIS.
- Review the appropriate report for the current status of the request:
- Students attending a District 75 program: Status of AT Evaluation (D75) Referrals
- All other students: Status of AT Evaluation (non-D75) Referrals
