Submitting a referral for an Assistive Technology evaluation in SESIS

When the Individualized Education Program (IEP) team determines that a student requires an assistive technology evaluation, the IEP team must add the evaluation to the student's Assessment Planning document. If there is not a New York City Department of Education (DOE) staff member at the school who is qualified to conduct the evaluation, the IEP team must submit a referral for an evaluation as follows:

To submit a referral for an Assistive Technology evaluation in SESIS

  1. In SESIS, locate the student's profile.
    How?

  2. From the Actions bar, click Documents.

    A list of all documents for the student appears.

  3. From the Create New Document drop-down menu, choose the appropriate document:

  4. For students attending a District 75 program: Choose Assistive Technology Evaluation Referral (D75).

  5. For all other students: Choose Assistive Technology Evaluation Referral (non-D75)

  6. Click the Go button, then click the Create button.

    The Request for Assistive Technology Evaluation form appears.

  7. Complete all relevant details.

  8. Click the Save, Done Editing button.

  9. From the Actions bar, click Change Status of This Document.

  10. From the Change Status from Draft to drop-down menu, choose Review.

  11. Click the Accept button.
