Completing an Assistive Technology evaluation
When an Individualized Education Program (IEP) team determines that a student requires an Assistive Technology evaluation, the IEP team must add the Assistive Technology evaluation to the Assessment Planning document.
If there is not an evaluator at the school who is available to conduct the evaluation, the IEP team must refer the student for evaluation as follows:
- Students attending a District 75 program must be referred to the Office of Technology Solutions (OTS).
- All other students must be referred to the Center of Assistive Technology (CAT).
To complete an Assistive Technology evaluation
- Ask the individual if there is an evaluator at the school who is able to complete the evaluation.

Yes, there is an evaluator available at the school |
- Open the Assessment Planning document.
- From the Assessments Needed drop-down menu, choose the appropriate value:
- For students attending a District 75 program: Assistive Technology for D75
- For all other students: Assistive Technology
- From the Evaluator drop-down menu, choose School Staff.
- Click the Save, Done Editing button.
The case manager will assign the evaluation to an evaluator.

- Submit a referral for an Assistive Technology evaluation.
How?
- Review the appropriate report for the current status of the referral:
- Students attending a District 75 program: Status of AT Evaluation (D75) Referrals report
- All other students: Status of AT Evaluation (non-D75) Referrals report
- Continue to check the status of the referral until OTS or CAT makes a decision on the request
- Once OTS or CAT makes a decision, complete the referral for an Assistive Technology evaluation.
