Completing a referral for an Assistive Technology evaluation

When the Individualized Education Program (IEP) team determines that a student requires an assistive technology evaluation and there is not a New York City Department of Education (DOE) staff member at the school who is qualified to conduct the evaluation, the IEP team must submit a referral for an evaluation as follows:

OTS or CAT reviews the referral and indicates their decision in SESIS. Once a decision is made, the IEP team must take the appropriate action.

To complete a referral for an Assistive Technology evaluation

  1. Review the appropriate report to determine the current status of the referral:

    If... Do this...
    Pending Review A decision has not been made on the referral.
    1. Continue to monitor the report for the most recent status of the referral.
    Returned to IEP Team OTS or CAT needs more information from the IEP team before making a decision on the referral.
    1. Open the Assistive Technology Evaluation Referral document and review the Additional Information field.

    2. Provide OST/CAT with the requested information.

    3. Change the status of the Assistive Technology Evaluation Referral document to Review.

      The status on the report is updated to Pending Review.

    4. Continue to monitor the report for the most recent status of the request.
    Approved The referral for an Assistive Technology evaluation was approved.
    1. Open the Assessment Planning document.

    2. From the Assessments Needed drop-down menu, choose the appropriate value:

      • For students attending a District 75 program, choose Assistive Technology for D75.

      • For all other students, choose Assistive Technology.

    3. From the Evaluator drop-down menu, choose Request for Assistance.

    4. Click the Save, Done Editing button.

      A representative from the Children First Network (CFN) will respond to the request and the site administrator will assign the case to the evaluator indicated by OTS or CAT.
    Disapproved The referral for an Assistive Technology evaluation was not approved.
    1. If the Assistive Technology evaluation was previously added to the Assessment Planning document, remove it.

    2. Make any necessary updates to the IEP document, if applicable.
