Adding the Level 1 Vocational assessment to the Assessment Planning document

The Level 1 Vocational assessment is a determination of a student's vocational skills, aptitudes, and interests. The assessment incorporates information gathered from the student, parents, guardians, teachers, and school records.

A Level 1 Vocational assessment is required for all initially referred students age 12 or older. It must be part of the information considered at the review of an Individualized Education Program (IEP) for all students receiving special education services who will turn 12 by the end of December in that school year. The Level 1 Vocational assessments are completed by the student, the teacher, and the parents or guardians.

When a student needs a Level 1 Vocational assessment, the assessments must be identified in the Assessment Planning document. When the evaluator meets with the teacher, parent or guardian and student to conduct the interviews, he or she must create the appropriate documents in the student's SESIS record to record the findings.

Important: SESIS prevents the user from creating the Level 1 Vocational documents if the evaluations are not identified in the Assessment Planning document.

Important: If the Level 1 Vocational assessment is required as part of an annual review, there will not be an Assessment Planning document. In this case, the student assessment must be done on paper and faxed into SESIS. The parent and teacher assessments are not required for an annual review.

To add the Level 1 Vocational assessment to the Assessment Planning document

  1. Determine which process stage the student is in.

  2. Select from below.

    If... Do this...
    Initial referral, reevaluation, or mandated three-year reevaluation
    1. Locate the student's profile.
      How?

    2. Click the documents () icon.

      A list of all documents for the student appears.

    3. Click Assessment Planning.

      Note: If the Assessment Planning document has not been created yet, create the document.

    4. From the Actions bar, click Edit this section.

    5. From the Assessment Needed drop-down menu, choose Level 1 Vocational - Student.

    6. From the Evaluator drop-down menu, choose the appropriate evaluator.

    7. From the Assessments Needed drop-down menu, choose Level 1 Vocational - Teacher.

    8. From the Evaluator drop-down menu, choose the appropriate evaluator.

    9. From the Assessment Needed drop-down menu, choose Level 1 Vocational - Parent/Guardian.

    10. From the Evaluator drop-down menu, choose the appropriate evaluator.

    11. Click the Save, Done Editing button.
    Annual review Since there is not an Assessment Planning document for an annual review, the Level 1 Vocational assessment must be completed outside of SESIS on the paper form and then faxed into the student's SESIS record using the appropriate fax coversheet.
    1. Complete the Level 1 Vocational assessment using the paper form.

    2. Locate the student's profile.
      How?

    3. Click the documents () icon.

      A list of all documents for the student appears.

    4. From the Create New Document drop-down menu, choose Documents Related to Assessments.

    5. Click the Go button.

    6. In the Comments field, type Level 1 Vocational - Student and click the Create button.

      The generic document appears.

    7. From the Actions bar, click Print.

    8. From the pop-up menu, choose FAX Coversheet.

      The fax coversheet appears.

    9. From the Category drop-down menu, choose Documents Related to Assessment.

    10. From the Form drop-down menu, choose Documents Related to Assessment.

    11. Click the calendar () icon and choose today's date.

    12. Click the Print button.

      The fax coversheet appears in Adobe Acrobat.

    13. In Acrobat, click the print () icon.

      The Print dialog box appears.

    14. Choose the appropriate printer and click the OK button.

      The fax coversheet prints.

    15. Fax the assessment and the coversheet into SESIS.
