Editing a finalized document
Once a document has been finalized, edits are not permitted. If a correction is required to a finalized document, a request must be made to have the status of the document changed from Final to Draft.
Some common reasons for edits include:
- To add an assessment to the Assessment Planning document.
Note: The assessment must have occurred previously. Individuals may not edit an Assessment Planning document to add a new assessment.
- To change a parent or guardian address on a notice that must be re-sent.
- To add additional notes from the social history meeting.
Edits may not materially change the document or the services provided to the student. If a change in service is required, a reevaluation must occur. If significant changes are required to a finalized IEP, an amendment may be appropriate.
Note: The request must be submitted for review.
To edit a finalized document
- Visit the SESIS Web site.
How?
Important: This is different than the SESIS application login page.
- On the main SESIS page, click the Getting Help link.
The Getting Help page appears.
- Scroll to the bottom of the page.
- In the SESIS Document Management Team section, click the link for the Document Control Request form.
The Web form appears.
- Complete the form.
- Click the Submit button.
You will be contacted when the request has been reviewed.
