Completing a Student Exit Summary

A Student Exit Summary must be completed for public school students and students who attend approved non-public schools whose eligibility for special education services terminate because they have reached the age of 21, or will receive a Regents, Local or IEP diploma.

The summary must be completed by a staff member who is familiar with the student's functional and academic levels, accomplishments, needs, necessary accommodations, and post-school goals.

The Student Exit Summary is not yet included in SESIS. The form can be located on page 224 of the Standard Operating Procedures Manual (SOPM) on the Department of Education Web site.

To complete a Student Exit Summary

  1. Open the SOPM.

  2. Go to page 224 and print the Student Exit Summary form.

  3. Complete the form.

  4. In SESIS, locate the student's profile.
    How?

  5. From the Actions bar, click Documents.

    A list of all documents for the student appears.

  6. From the create New Document drop-down menu, choose Documents Related to IEP.

  7. Click the Go button.

  8. In the Comment field, type Student Exit Summary.

  9. Click the Create button.

  10. From the Actions bar, click Print.

  11. From the pop-up menu, choose Fax Coversheet.

  12. From the Category drop-down menu, choose Documents Related to IEP.

  13. From the Form drop-down menu, choose Documents Related to IEP.

  14. Next to the Date Received field, click the calendar () icon and choose today's date.

  15. Click the Print button.

    The fax coversheet appears in Adobe Acrobat.

  16. In Acrobat, click the Print () icon.

    The Print dialog box appears.

  17. Choose the appropriate printer and click the OK button.

    The fax coversheet prints.

  18. Fax the coversheet with the completed Student Exit Summary form into SESIS.

  19. Click the browser's back button.

  20. From the Actions bar, click Documents related to IEP.

  21. Finalize the document.
