Logging outreach attempts in SESIS for passive consent

Before a student may be evaluated for special education services, his or her parents or guardians must provide consent. When the evaluation is conducted as part of a reevaluation, the evaluation may occur without written consent from the parents or guardians as long as three outreach attempts have been recorded in the student's event log in SESIS. This is referred to as passive consent.

Important: Passive consent is not acceptable for initial referral cases. The student's parents or guardians must provide written consent for an initial referral.

The three outreach attempts include the automatic event that is recorded in the student's event log when printing and sending the Consent for Additional Assessments (New Testing Needed) letter as well as two additional manual events (or three manual events if the letter was not printed and sent) recording the individual's attempt to contact the parents or guardians, such as by phone or additional letters sent to the home address.

If the parents or guardians provide written consent, the signed consent form must be faxed into SESIS using the appropriate fax coversheet.

Important: If a parent or guardian refuses consent, the child may not be evaluated.

To log outreach attempts in SESIS for passive consent

  1. Verify that the parents or guardians did not refuse consent.

    Important: If the parents or guardians refused consent, the child may not be evaluated without an impartial hearing.

  2. If the Consent for Additional Assessment (New Testing Needed) letter was not printed and mailed to the parent or guardian, send the letter and make the necessary outreach attempts (as described on page 34 of the Standard Operating Procedures Manual). Once all outreach attempts have been logged in the student's event log, the Assessment Planning document may be modified.

    Important: SESIS will not allow you to move forward unless the consent letter has been sent (and the user clicked Send/Print and accepted the prompt to record this as an event) and two outreach attempts have been made at least one day apart.

  3. In SESIS, locate the student's profile.
    How?

  4. From the Actions bar, click Events.

    A list of all events for the student's case appears.

  5. From the Actions bar, click Add New Event.

  6. In the Event Description field, type a description for the event.

  7. Next to the Date And Time Of Event field, click the calendar () icon and choose the date of the outreach attempt.

  8. Type the time of the outreach attempt.

  9. From the Response Type drop-down menu, choose the appropriate response.

  10. From the Log Type drop-down menu, choose the appropriate value.

  11. From the Link event to document drop-down menu, choose Consent for Additional Assessments.

    Important: If this step is skipped, the event will not be linked to the Consent for Additional Assessments document and will not be counted as an outreach attempt for passive consent.

  12. Click the Accept button.
