Creating an initial referral

When a referral for evaluation comes directly from the parent or guardian, an Initial Referral document must be created and the request must be faxed into SESIS.

Note: At any point, you may save a draft of this document and complete it at a later time.

To create an initial referral

  1. In SESIS, locate the student's profile.
    How?

    Tip: If the student does not yet have a profile in SESIS, you must create a profile for the student.

  2. Click the Documents link.

  3. From the Create New Document drop-down menu, choose Initial Referral.

    Note: If the referral for evaluation came from anyone other than the parent or guardian, choose Request for Initial Referral.

  4. Click the Go button.

  5. If applicable, type a comment in the Comment field.

  6. Click the Create button.

    The Initial Referral form appears.

  7. From the Actions bar, click Fax Coversheet.

  8. From the Category drop-down menu, choose Initial Referral.

  9. From the Form drop-down menu, choose the appropriate type.

    Note: If the referral came from the parent or guardian, choose Initial Referral - Parent. If the request came from the principal or the Committee on Special Education (CSE) chairperson, choose Initial Referral - Principal; Chairperson.

  10. Click the calendar () icon and choose the date the referral was received.

  11. From the Title of Person Making Request drop-down menu, choose the appropriate title.

  12. In the Name of Person Making Request for Referral field, type the name of the person who made the request.

  13. Click the Print button.

    The Initial Referral fax coversheet appears as a PDF.

  14. In Acrobat, click the print () icon.

  15. Choose the appropriate printer and click the OK button.

    The Initial Referral fax coversheet prints.

  16. Attach the Initial Referral fax coversheet to the Initial Referral and fax both pages to the number located in the upper left corner of the fax coversheet.

  17. Save the paper copy of the initial referral and the fax coversheet in a file for the student.

  18. Click the browser's back button.

    Important: This is the only scenario where it is appropriate to use the browser's back button.

  19. From the Actions bar, click Initial Referral.

  20. From the Actions bar, click the Select drop-down () icon.

  21. From the pop-up menu, choose Procedural Safeguards Notice.

    The Procedural Safeguards Notice appears.

  22. Click the Save, Done Editing button.

  23. Click the workflow prompt under the Actions bar to finalize the document.

  24. From the Change status from Draft to drop-down menu, choose Final.

  25. Click the Accept button.

  26. Click the OK button to change the status to Final.

  27. Send the Notice of Referral: Initial.
