Completing the physical examination
A physical examination is required for all initial special education evaluation cases. The student's doctor must complete the Physical Examination form and return it to the student's parent or guardian.
The completed and signed Physical Examination form must be faxed into SESIS with the appropriate fax coversheet.
To complete the physical examination
- Obtain the completed form from the parent or guardian.
Note: The Physical Examination form was distributed to the parent or guardian as part of the Social History package.
- Locate the student's profile.
How?
- From the Actions bar, click Documents.
A list of all documents for the student appears.
- From the Create New Document drop-down menu, choose Physical Examination from the Referral section.
- Click the Go button.
- Click the Create button.
The Physical Examination form appears in Edit mode.
- Click the Cancel button.
- From the Actions bar, click Print.
- From the pop-up menu, click FAX Coversheet.
The fax coversheet page appears.
- From the Category drop-down menu, choose Physical Examination.
- From the Form drop-down menu, choose Request for Physical Examination.
- Click the calendar (
) icon next to the Date Received field and choose the date the completed form was received.
- Click the calendar (
) icon next to the Date of Physical Exam field and choose the date of the exam.
- Click the Print button.
The fax coversheet appears in Adobe Acrobat.
- In Acrobat, click the print (
) icon.
The Print dialog box appears.
- Choose the appropriate printer and click the OK button.
The fax coversheet prints.
- Fax the coversheet and the signed Physical Examination form to the number provided on the coversheet.
