Recording consent for initial evaluation for special education services in SESIS

When a parent or guardian signs the consent form to allow his or her child to be evaluated for special education services, the form must be faxed into SESIS. The student's profile is then updated to reflect that consent has been provided or denied.

Note: The assessment planning process cannot proceed before consent is obtained.

To record consent for evaluation for special education services in SESIS

  1. In SESIS, locate the student's profile.
    How?

  2. From the Actions bar, click Documents, then click Social History Package.

  3. From the Actions bar, click Print.

  4. From the pop-up menu, click Fax Coversheet.

  5. From the Category drop-down menu, choose Social History Package.

  6. From the Form drop-down menu, choose Consent for Initial Evaluation.

  7. In the Date Received field, click the calendar () icon and choose the date the consent form was returned.

  8. Indicate whether the parent consented to evaluation.

  9. Click the Print button.

    The fax coversheet appears.

  10. From the File menu, choose Print.

  11. In the Print dialog box, choose the appropriate printer, and then click the OK button.

  12. Close the Adobe window.

  13. Click the browser back button.

    Note: This is the only situation where it is OK to click the back button in SESIS.

  14. If additional consent forms were returned, repeat this procedure for each additional form.

  15. Fax the coversheet and the signed consent form to the fax number provided on the coversheet.

  16. Save the coversheet and the form in a file folder for future reference.

  17. Create the Assessment Planning document.
