Preparing the Social History package

When a child is referred for an evaluation for special education services and the referral is approved by the principal or CSE chairperson, the New York City Department of Education (DOE) prepares a Social History package for the child's parent or guardian..

To prepare the Social History package

  1. Send the Notice of Social History.
    How?

  2. From the SESIS home page, in the Reports section, click Status of Initial Referrals.

  3. Locate the student's name from the list and click the magnifying glass icon.

    The student profile page appears.

    Tip: You may also search for the student's profile to access the student's document page.

  4. Click the Documents link.

  5. From the Create New Document drop-down menu, choose Social History Package.

  6. Click the Go button.

  7. Click the Create button.

  8. In the Date field, verify that today's date is populated.

    Note: If you are not sending the package on this date, change the date to the correct date.

  9. Verify that the family's contact information is accurate and make any necessary adjustments.

  10. Click the Save, Done Editing button.

  11. Review the workflow prompts at the top of the page to determine if any additional documents are needed. These can be completed during the social history meeting.

    Note: Any additional required documents are determined based on the student's profile. For example, if the parent's primary language is not English, the Home Language form must be completed.

  12. Click the Print icon.

  13. From the pop-up menu, click All Sections.

  14. Choose the appropriate printer and click the Print button.

  15. Click the Return to previous screen link.
