Faxing a student prescription into SESIS

A physician's prescription is required when making a recommendation for occupational or physical therapy. The prescription must be faxed into SESIS using the Documents Related to IEP fax coversheet.

To fax a student prescription into SESIS

1. In SESIS, locate the student's Documents page.
   
   
2. From the the IEP section of the Create New Document dropdown list, select Documents Related to IEP
   
   A form appears.
   
   
3. From the Select a Template drop-down menu, scroll to the IEP section and then choose Documents Related to IEP and click the Go button.
   
   The form refreshes with a Comment box.
   
   
4. Type "prescription for" and the relevant service.
   
   
5. Click the Create button
   
   The form refreshes.
   
   
6. From the Actions toolbar, select Print, and then FAX Coversheet.
   
   The form refreshes.
   
   
7. From the Category dropdown menu, select Documents Related to IEP.
   
   
   
8. From the Form dropdown menu, select Documents related to IEP.
   
   The form refreshes.
   
   
9. Click the calendar icon to select a date for the Date Received field. Select the date that the prescription was received by the Department of Education.
   
   
10. Click the Print button.
    
    The Windows Print dialog box appears. Make any needed adjustments.
    
    
11. Fax the prescription with the fax coversheet into SESIS.
