Creating a request for initial referral

When you suspect that a student has a disability and may require special education services, you must create a request for initial referral in SESIS.

Note: At any point, you may save a draft of this document and complete it at a later time.

To create a request for initial referral

  1. In SESIS, locate the student's profile.
    How?

    Tip: If the student profile is not located, create a profile for the student.

  2. Click the Documents link.

  3. From the Create New Document drop-down menu, choose Request for Initial Referral.

    Note: If the referral for evaluation came directly from the parent or guardian, and you need to fax in the request, choose Initial Referral.

  4. Click the Go button.

  5. If applicable, type a comment in the Comment field.

  6. Click the Create button.

    The Request for Initial Referral form appears.

  7. Verify that the name in the Initial Referral Source Name field is correct.

    Note: The field will be pre-populated with your name. If there is an error, contact the individual at your school who is responsible for updating ATS.

  8. Verify the parent or guardian's name.

    Note: The pre-populated data is from ATS. If changes are required, the parent or guardian must contact the school to update ATS.

  9. Complete the Demographic Data section. Be sure to note the following:

  11. Complete the Intervention Strategies section. Be sure to note the following:

  12. In the Conferences with Parent/Guardian section, type the dates of all parent conferences held with the parent or guardian to discuss school-related performance.

    Important: At least one entry is required.

  13. If an intervention type was not entered in the Intervention Strategies section, type an explanation in the Instructional/Programmatic Interventions field, located in the Instructional/Programmatic Interventions section.

    Important: This field is required if no intervention strategies were indicated in the Intervention Strategies section.

  14. Click the Save, Done Editing button.

    Note: If any errors were made or any required fields left blank, an error message appears at the top of the page.

  15. Click the link to submit the form for approval.

  16. Click the Accept button.

  17. Click the OK button to change the status of the document from Draft to Review.
