Creating a request for initial referral
When you suspect that a student has a disability and may require special education services,
you must create a request for initial referral in SESIS.
Note: At any point, you may save a draft
of this document and complete it at a later time.
To create a request for initial referral
- In SESIS, locate the student's profile.
How?
Tip: If the student profile is not located,
create a profile for the student.
- Click the Documents link.
- From the Create New Document drop-down menu, choose Request for Initial Referral.
Note: If the referral for evaluation came directly from the parent or guardian, and you need to fax in the request, choose Initial Referral.
- Click the Go button.
- If applicable, type a comment in the Comment field.
- Click the Create button.
The Request for Initial Referral form appears.
- Verify that the name in the Initial Referral Source Name field is correct.
Note: The field will be pre-populated with your name. If there is an error, contact the individual at your school who is responsible for updating ATS.
- Verify the parent or guardian's name.
Note: The pre-populated data is from ATS. If changes are required, the parent or guardian must contact the school to update ATS.
- Complete the Demographic Data section. Be sure to note the following:
- The Language student speaks/understands field is required.
- The State the reason for referral/specific nature of the problem field is required.
- Complete the Background Information section. Be sure to note the following:
- Upload a copy of the most recent results of the students hearing and vision screening.
- If the student is an English Language Learner (ELL), indicate the grade level at which the student is currently functioning when performing in his or her native language as well as in English.
- Complete the Intervention Strategies section. Be sure to note the following:
- At least one intervention is required. If no intervention strategies were initiated, type a reason in the Instructional/Programmatic Interventions section.
- For each intervention strategy listed, type the initiation and termination dates. If the intervention is ongoing, leave the termination date blank and indicate that it is an ongoing intervention in the Description field.
- A description of each intervention strategy indicated is required. The field is not labeled as required.
- Use the lookup link to choose a provider name. If the provider is not listed, type his or her full name and title in the appropriate fields.
- In the Conferences with Parent/Guardian section, type the dates of all parent conferences held with the parent or guardian to discuss school-related performance.
Important: At least one entry is required.
- If an intervention type was not entered in the Intervention Strategies section, type an explanation in the Instructional/Programmatic Interventions field, located in the Instructional/Programmatic Interventions section.
Important: This field is required if no intervention strategies were indicated in the Intervention Strategies section.
- Click the Save, Done Editing button.
Note: If any errors were made or any required fields left blank, an error message appears at the top of the page.
- Click the link to submit the form for approval.
- Click the Accept button.
- Click the OK button to change the status of the document from Draft to Review.
